• Time for Q&A and guidance on how to move forward in your wellness journey

Membership Requirement

• A $40 annual membership is required to establish your client file and receive ongoing access to member benefits, including discounts, educational resources, and follow-up support.

Paperwork

• Please complete the new client intake and membership forms before your appointment.

• These forms provide your health history and consent, allowing Sherri to prepare and maximize your consult time.

• Paperwork must be submitted prior to or at the beginning of your consult.
