Membership Requirement

• A $40 annual membership is required to establish your client file and receive ongoing access to member benefits, including discounts, educational resources, and follow-up support.

• Please complete the new client intake and Membership forms before your appointment.

• These forms provide your health history and consent, allowing Sherri to prepare and maximize your consult time.

• Paperwork must be submitted before your consult.
